LCdr Mitchell Drake is the RCN\u2019s Fleet Surgeon on the west coast.
SOUMIS<\/figcaption><\/figure>\nAmidst a global pandemic, this core philosophy of the Canadian Armed Forces\u00a0is as important now as it has ever been. The objective to preserve Force health while\u00a0continuing Force generation\u00a0lies at the forefront of the minds,\u00a0and\u00a0ultimately\u00a0in the hands,\u00a0of\u00a0military leadership\u00a0and medical\u00a0professionals.<\/p>\n
The month\u00a0of December saw several warships and hundreds of sailors at sea\u00a0off the coast of British Columbia participating in\u00a0Exercise\u00a0Trident Fury,\u00a0all\u00a0while the province experienced a resurgence in COVID-19 cases and\u00a0issued\u00a0restrictions to\u00a0certain\u00a0activities.\u00a0The\u00a0capability to exercise the fleet under these\u00a0conditions can be accredited to the efforts of\u00a0many, including\u00a0Fleet Surgeon\u00a0LCdr Mitchell Drake.<\/p>\n
LCdr\u00a0Drake\u00a0shares the same position\u00a0former Medical Officer now Provincial Health Officer, Dr. Bonnie Henry\u00a0once held during her time in the Forces.\u00a0In\u00a0light of the current pandemic, he now finds himself liaising with her and her staff regularly.\u00a0Both east coasters\u00a0with hometowns in PEI,\u00a0and having attended Dalhousie University in Halifax, N.S.,\u00a0for medical school,\u00a0LCdr\u00a0Drake jokingly remarks\u00a0of the commonalities. \u201cI don\u2019t\u00a0have any shoes named after\u00a0me\u00a0yet, but there\u2019s still time.\u201d<\/p>\n
His career began in the Naval Reserves\u00a0serving as a Logistics Officer while studying to become a physician.\u00a0Since\u00a0completing his residency in Calgary, AB,\u00a0he\u00a0has served in a\u00a0variety of roles,\u00a0from deployments\u00a0with\u00a0the Canadian Army\u00a0in Europe\u00a0and the Royal Canadian Navy\u00a0at sea in\u00a0Asterix,\u00a0to more clinical settings as\u00a0Base Surgeon and\u00a0now\u00a0Fleet Surgeon.\u00a0\u00a0<\/span><\/p>\nIn March, about a week before the Base introduced COVID-19 protocols, he started his current position; his responsibilities are extensive.<\/span><\/p>\n\u201cMy main role\u00a0is advising Command Staff in the Fleet, lately\u00a0on the topic of\u00a0COVID-19, as well as being the senior medical authority on this coast for all of the operational clinicians.\u201d<\/p>\n
\u00a0<\/span>To meet the unique challenges\u00a0facing military operations, they are using\u00a0tools specifically developed for the Canadian Armed Forces\u00a0and now more recently for the\u00a0navy.\u00a0\u00a0<\/span><\/p>\n\u201cWe\u2019ve leveraged our scientists at\u00a0Defence\u00a0Research Development Canada, who have developed excellent modelling tools and maps. We use these and other\u00a0risk calculators they\u2019ve developed to determine the likelihood of bringing a case of COVID-19 on board a ship or into a unit, based on the prevalence of infection in the relevant community,\u00a0 the length and type\u00a0of\u00a0quarantine,\u00a0combined with\u00a0testing strategies. I do a lot of those calculations in order to\u00a0advise on risk. While the virus is circulating in our community, we\u2019re not going to get to a place where the risk is zero when embarking a ship or cohorting a large group, but we need to mitigate the risk to a reasonable level given the task they\u2019re going out to do.\u201d<\/p>\n
\u00a0<\/span>He\u00a0remains\u00a0up-to-date on\u00a0the\u00a0latest evidence\u00a0to determine transmission risks,\u00a0to\u00a0develop protocols for ships at sea should they have suspicious cases of respiratory illness present to the deployed medical team, or\u00a0to\u00a0determine how\u00a0to overcome an outbreak scenario. But his\u00a0primary goal\u00a0is\u00a0preventing such scenarios\u00a0through\u00a0risk mitigation strategies,\u00a0including pre-embarkation protocols and testing.\u00a0\u00a0<\/span><\/p>\nUnits are following\u00a0one of three\u00a0pre-embarkation quarantine\u00a0protocols\u00a0prior to sailing:\u00a0CAF and provincial guidance with travel and group restrictions added;\u00a0a modified quarantine;\u00a0or a full quarantine.<\/p>\n
Modified quarantines\u00a0and\u00a0full quarantines\u00a0typically last\u00a0seven to\u00a014 days, occur in a member\u2019s home or military provided accommodations, and come with specific\u00a0restrictions.\u00a0Modified quarantines\u00a0allow specific essential activities to be conducted, and the potential for members to spend the quarantine period in their homes should set household requirements be met.<\/p>\n
The strict protocol of\u00a0a\u00a0full quarantine\u00a0eliminates all interactions with others.<\/p>\n
LCdr\u00a0Drake emphasizes the importance that\u00a0members follow\u00a0their assigned quarantine\u00a0protocol.<\/p>\n
\u201cWe\u2019re putting a\u00a0lot of trust in the sailors. Where we can,\u00a0we want to allow for a\u00a0quarantine\u00a0that still\u00a0gives members the opportunity to\u00a0spend time at home and with their families,\u00a0if possible,\u00a0prior to sailing. With the increase in\u00a0prevalence\u00a0on Vancouver Island lately we are forced to consider full quarantine as the primary protocol. We\u2019re going through the tabletop exercises now to determine what triggers specifically indicate the requirement for full quarantine. We\u2019re recalculating risk on a daily basis. The Commodore and I are very hopeful British Columbians will continue to heed the advice of Dr. Henry and that we\u2019ll see a significant reduction in our prevalence on the Island such that we can limit the length of quarantine and its impact on our sailors and their families.\u201d<\/span><\/p>\nPrior to Exercise Trident Fury,\u00a0with\u00a0less than two weeks separating their departures,\u00a0members\u00a0of\u00a0HMCS\u00a0<\/span>Calgary\u00a0<\/span><\/i>and\u00a0HMCS<\/span>\u00a0Regina\u00a0<\/span><\/i>conducted\u00a0seven\u00a0and\u00a010 day\u00a0quarantine periods\u00a0respectively, and were required to pass two rounds of COVID-19 testing prior to embarkation, to combat the increasing risk.\u00a0\u00a0<\/span><\/p>\nMembers also completed self-assessments and were screened for personal risk factors prior to embarkation.\u00a0Once on\u00a0board\u00a0ship,\u00a0masks are worn for a minimum of\u00a0four\u00a0days, relaxing the use of\u00a0non-medical masks\u00a0after four days,\u00a0at the Commanding Officer\u2019s discretion.\u00a0<\/span>Once a ship is underway, the minimum requirements for a member to join a ship is seven\u00a0days\u00a0<\/span>full\u00a0quarantine<\/span>\u00a0and a negative COVID-19\u00a0test.<\/span><\/p>\nIn total,\u00a0amidst the surge in prevalence in the province,\u00a0an estimated\u00a01,400 tests will be conducted in the\u00a0two\u00a0weeks leading up to\u00a0the exercise.<\/p>\n
\u201cWe don\u2019t have intrinsic testing capabilities here in Esquimalt in our clinic yet.\u00a0All\u00a0of the testing that\u2019s happened ashore has relied on Island Health and their lab staff at Victoria\u00a0General\u00a0Hospital. Despite the pre-existing workloads they face, they\u2019ve\u00a0been eager, exceptionally helpful and flexible\u00a0in\u00a0supporting\u00a0us,\u201d says\u00a0LCdr\u00a0Drake.\u00a0\u201cIt\u2019s been an interesting time. Scientists,\u00a0clinicians and\u00a0Commanders\u00a0have had to\u00a0collaborate closely in ways I\u2019ve previously never seen\u00a0in my career.\u201d<\/p>\n
