Par la Capt Christine MacNeil<\/span><\/b>\u00a0<\/span><\/p>\nLe Mat 1 Kyle Curlett, membre de l\u2019\u00e9quipe de d\u00e9pistage des bless\u00e9s, observe un coll\u00e8gue qui se pr\u00e9par \u00e0 simuler l\u2019injection d\u2019un bless\u00e9 dans le cadre d\u2019un exercice de protocole de transfusion massive, \u00e0 bord du NCSM Ville de Qu\u00e9bec, le 15 mai. LE CPL BRENDAN GAMACHE<\/figcaption><\/figure>\n
Le Navire canadien de Sa Majest\u00e9 (NCSM) Ville de Qu\u00e9bec est pass\u00e9 par la mer Rouge et le d\u00e9troit de Bab el Mandeb alors qu\u2019il faisait le tour du monde pendant son d\u00e9ploiement dans le cadre de l\u2019op\u00e9ration HORIZON. Ce trajet n\u2019est pas entrepris \u00e0 la l\u00e9g\u00e8re, car il est consid\u00e9r\u00e9 comme une des voies navigables les plus risqu\u00e9es au monde. Comme c\u2019est l\u2019habitude dans les FAC, le NCSM Ville de Qu\u00e9bec esp\u00e9rait le mieux et planifiait pour le pire. Un de ces plans \u00e9tait d\u2019assigner un m\u00e9decin militaire g\u00e9n\u00e9raliste (MMG) \u00e0 bord pour venir en aide \u00e0 l\u2019\u00e9quipe m\u00e9dicale du navire pour cette partie du voyage.<\/p>\n
Le MMG, conscient que les \u00e9quipes m\u00e9dicales du navire ont g\u00e9n\u00e9ralement une exp\u00e9rience limit\u00e9e dans la manipulation du sang, a d\u00e9cid\u00e9 d\u2019organiser une simulation de transfusion de sang massive en utilisant les unit\u00e9s de sang \u00e0 bord une fois la date de p\u00e9remption d\u00e9pass\u00e9e.<\/span>\u00a0<\/span><\/p>\n
En collaboration avec les Services de sant\u00e9 des Forces canadiennes, il a cr\u00e9\u00e9 un sc\u00e9nario qui \u00e9tablirait une norme en mati\u00e8re d\u2019instruction pour l\u2019avenir. Des listes de v\u00e9rification et des lignes directrices pour la manipulation en toute s\u00e9curit\u00e9 du sang ont \u00e9t\u00e9 incorpor\u00e9es, et les membres de la cha\u00eene de commandement m\u00e9dicale \u00e9taient tr\u00e8s enthousiastes \u00e0 l\u2019id\u00e9e de cette occasion. Les participants \u00e0 cette instruction comprenaient les bless\u00e9s fictifs, l\u2019AM (ou \u00ab doc du navire \u00bb) et l\u2019ambulancier param\u00e9dical (\u00ab b\u00e9b\u00e9 doc \u00bb), ainsi que deux volontaires de l\u2019\u00e9quipe d\u2019\u00e9vacuation des bless\u00e9s, le tout sur la supervision du MMG.<\/span>\u00a0<\/span><\/p>\n
Le sc\u00e9nario a commenc\u00e9 par le son de l\u2019alarme \u00ab des bless\u00e9s, des bless\u00e9s, des bless\u00e9s \u00bb dans tout le navire. Les deux responsables de l\u2019\u00e9vacuation des bless\u00e9s ont \u00e9t\u00e9 les premiers r\u00e9pondants et ont trouv\u00e9 un bless\u00e9 simul\u00e9 dont le poignet aurait \u00e9t\u00e9 partiellement amput\u00e9 par un panneau qui s\u2019\u00e9tait referm\u00e9 inopin\u00e9ment. Le bless\u00e9 a \u00e9t\u00e9 \u00e9valu\u00e9 rapidement, puis emmen\u00e9 \u00e0 l\u2019infirmerie pour traitement. Apr\u00e8s une \u00e9valuation plus compl\u00e8te par l\u2019AM, il a \u00e9t\u00e9 d\u00e9termin\u00e9 qu\u2019une transfusion de sang \u00e9tait n\u00e9cessaire, et l\u2019\u00e9quipe a install\u00e9 le mat\u00e9riel n\u00e9cessaire dans l\u2019infirmerie du navire pour r\u00e9aliser cette proc\u00e9dure (toujours simul\u00e9e).<\/span>\u00a0<\/span><\/p>\n
Apr\u00e8s avoir simul\u00e9 l\u2019insertion d\u2019un dispositif intraveineux (IV) dans le bless\u00e9, le sang a \u00e9t\u00e9 plac\u00e9 dans un r\u00e9chauffeur pour atteindre la temp\u00e9rature appropri\u00e9e et inject\u00e9 dans le bless\u00e9, que l\u2019on observait attentivement pour d\u00e9tecter tout changement dans son \u00e9tat. Une r\u00e9action n\u00e9gative a \u00e9t\u00e9 observ\u00e9e et l\u2019\u00e9quipe a rapidement d\u00fb cesser la transfusion pour traiter les sympt\u00f4mes. Une fois le patient stabilis\u00e9, la seconde poche de sang a \u00e9t\u00e9 plac\u00e9e et la proc\u00e9dure r\u00e9p\u00e9t\u00e9e. Avec la transfusion de deux poches de sang (dans une bouteille vide, alors que le \u00ab dispositif IV \u00bb \u00e9tait dans le bras du patient), l\u2019\u00e9quipe du VDQ est devenue la premi\u00e8re de l\u2019histoire r\u00e9cente de la MRC \u00e0 suivre cette instruction.<\/p>\n
L\u2019ajout de cette instruction \u00e0 l\u2019arsenal m\u00e9dical du navire accorde une nouvelle capacit\u00e9 \u00e0 l\u2019infirmerie. La capacit\u00e9 de transfuser du sang dans un patient en h\u00e9morragie permettra \u00e0 l\u2019\u00e9quipe m\u00e9dicale de prolonger la p\u00e9riode entre la blessure et l\u2019\u00e9vacuation vers un niveau de soins sup\u00e9rieur, un \u00e9l\u00e9ment critique lors des op\u00e9rations dans des environnements isol\u00e9s et aust\u00e8res. Le VDQ a \u00e9t\u00e9 le premier navire \u00e0 suivre cette instruction, mais maintenant que les lignes directrices sont \u00e9tablies et que les avantages de cette instruction sont \u00e9vidents, il ne sera s\u00fbrement pas le dernier.<\/p>\nLe Cpl Dakota Lacroix inspecte la poche de sang qui sera utilis\u00e9e pour mener un exercice de protocole de transfusion massive.
